Título

Bilateral Central Retinal Vein Occlusion and Coronavirus disease

Objetivo

To describe a rare case of bilateral simultaneous central retinal vein occlusion (CRVO) after coronavirus disease, seven days after RT-PCR positive for SARS-Cov 2.

Relato do Caso

A 51-year-old black female presented with decreased vision in both eyes, simultaneously, fourteen days after fever and sore throat and seven days after a positive RT-PCR SARS-cov 2 test. History of mild systemic hypertension. Ocular examination showed intraretinal hemorrhages in both eyes and macular edema on optical coherence tomography, compatible with bilateral central retinal vein occlusion. Laboratory data revealed high levels of D-Dimer erythrocyte sedimentation, ferritin and PCR ultra sensitive. The patient received anti-vascular endothelial growth factor therapy, with anatomic and functional improvement.

Conclusão

The prevalence rate of bilateral CRVO is rare. We report a rare case of simultaneous bilateral CRVO after SARS -Cov 2 infection. Retinal findings following infection with SARS-Cov 2 virus may occur due to complement activated thrombotic microangiopathy and hypercoagulable state, that can cause retinal artery and venous occlusion.
The evidence of the correlation of CRVO and coronavirus disease is still controversial, but ophthalmologists must be aware of this potential complication.
